Adoption Consent Form

An Adoption Consent Form is a legal document in which a biological parent (or parents) voluntarily agrees to the adoption of their child by another person or couple. This consent is a critical requirement in adoption proceedings and must be given voluntarily and with full understanding of the consequences. The form typically states that the parent understands they are permanently relinquishing all parental rights and responsibilities. Most jurisdictions require a waiting period after birth before consent can be given, and some allow a revocation period during which consent can be withdrawn. The consent must usually be witnessed, notarized, or executed before a judge. In some cases, such as stepparent adoptions or when parental rights have been terminated, consent may not be required from the biological parent.
